Hospitals and injury are always such a staple of angst fics, but 9 times out of 10 the author has clearly never been in an emergency situation and the scenes always come off as over-dramatized and completely unbelievable. So here’s a crash course on hospital life and emergencies for people who want authenticity. By someone who spends 85% of her time in a hospital.
Lights and sirens are usually reserved for the actively dying. Unless the person is receiving CPR, having a prolonged seizure or has an obstructed airway, the ambulance is not going to have lights and sirens blaring. I have, however, seen an ambulance throw their lights on just so they can get back to the station faster once. Fuckers made me late for work.
Defibrillators don’t do that. You know, that. People don’t go flying off the bed when they get shocked. But we do scream “CLEAR!!” before we shock the patient. Makes it fun.
A broken limb, surprisingly, is not a high priority for emergency personnel. Not unless said break is open and displaced enough that blood isn’t reaching a limb. And usually when it’s that bad, the person will have other injuries to go with it.
Visitors are not generally allowed to visit a patient who is unstable. Not even family. It’s far more likely that the family will be stuck outside settling in for a good long wait until they get the bad news or the marginally better news. Unless it’s a child. But if you’re writing dying children in your fics for the angst factor, I question you sir.
Unstable means ‘not quite actively dying, but getting there’. A broken limb, again, is not unstable. Someone who came off their motorbike at 40mph and threw themselves across the bitumen is.
CPR is rarely successful if someone needs it outside of hospital. And it is hard fucking work. Unless someone nearby is certified in advanced life support, someone who needs CPR is probably halfway down the golden tunnel moving towards the light.
Emergency personnel ask questions. A lot of questions. So many fucking questions. They don’t just take their next victim and rush off behind the big white doors into the unknown with just a vague ‘WHAT HAPPENED? SHE HIT HER HEAD?? DON’T WORRY SIR!!!’ They’re going to get the sir and ask him so many questions about what happened that he’s going to go cross eyed. And then he’s going to have to repeat it to the doctor. And then the ICU consultant. And the police probably.
In a trauma situation (aka multiple injuries (aka car accident, motorbike accident, falling off a cliff, falling off a horse, having a piano land on their head idfk you get the idea)) there are a lot of people involved. A lot. I can’t be fucked to go through them all, but there’s at least four doctors, the paramedics, five or six nurses, radiographers, surgeons, ICU consultants, students, and any other specialities that might be needed (midwives, neonatal transport, critical retrieval teams etc etc etc). There ain’t gonna be room to breathe almost when it comes to keeping someone alive.
Emergency departments are a life of their own so you should probably do a bit of research into what might happen to your character if they present there with some kind of illness or injury before you go ahead and scribble it down.
Nurses run them. No seriously. The patient will see the doctor for five minutes in their day. The nurse will do the rest. Unless the patient codes.
There is never a defibrillator just sitting nearby if a patient codes.
And we don’t defibrillate every single code.
If the code does need a defibrillator, they need CPR.
And ICU.
They shouldn’t be on a ward.
There are other people who work there too. Physiotherapists will always see patients who need rehab after breaking a limb. Usually legs, because they need to be shown how to use crutches properly.
Wards are separated depending on what the patient’s needs are. Hospitals aren’t separated into ICU, ER and Ward. It’s usually orthopaedic, cardiac, neuro, paediatric, maternity, neonatal ICU, gen surg, short stay surg, geriatric, palliative…figure out where your patient is gonna be. The care they get is different depending on where they are.
A patient is only in ICU if they’re at risk of active dying. I swear to god if I see one more broken limb going into ICU in a fic to rank up the angst factor I’m gonna shit. It doesn’t happen. Stop being lazy.
Tubed patients can be awake. True story. They can communicate too. Usually by writing, since having a dirty great tube down the windpipe tends to impede ones ability to talk.
The nursing care is 1:1 on an intubated patient. Awake or not, the nurse is not gonna leave that room. No, not even to give your stricken lover a chance to say goodbye in private. There is no privacy. Honestly, that nurse has probably seen it all before anyway.
ICU isn’t just reserved for intubated patients either. Major surgeries sometimes go here post-op to get intensive care before they’re stepped down. And by major I mean like, grandpa joe is getting his bladder removed because it’s full of cancer.
Palliative patients and patients who are terminal will not go to ICU. Not unless they became terminally ill after hitting ICU. Usually those ones are unexpected deaths. Someone suffering from a long, slow, gradually life draining illness will probably go to a general ward for end of life care. They don’t need the kind of intensive care an ICU provides because…well..they’re not going to get it??
No one gets rushed to theatre for a broken limb. Please stop. They can wait for several days before they get surgery on it.
Honestly? No one gets ‘rushed’ to theatre at all. Not unless they are, again, actively dying, and surgery is needed to stop them from actively dying.
Except emergency caesarians. Them babies will always get priority over old mate with the broken hip. A kid stuck in a birth canal and at risk of death by pelvis is a tad more urgent than a gall stone. And the midwives will run. I’ve never seen anyone run as fast as a midwife with a labouring woman on the bed heading to theatres for an emergency caesar.
Surgery doesn’t take as long as you think it does. Repairing a broken limb? Two hours, maybe three tops. Including time spent in recovery. Burst appendix? Half an hour on the table max, maybe an hour in recovery. Caesarian? Forty minutes or so. Major surgeries (organs like kidneys, liver and heart transplants, and major bowel surgeries) take longer.
You’re never going to see the theatre nurses. Ever. They’re like their own little community of fabled myth who get to come to work in their sweatpants and only deal with unconscious people. It’s the ward nurse who does the pick up and drop offs.
Anyway there’s probably way, way more that I’m forgetting to add but this is getting too long to keep writing shit. The moral of the story is do some research so you don’t look like an idiot when you’re writing your characters getting injured or having to be in hospital. It’s not Greys Anatomy in the real world and the angst isn’t going to be any more intense just because you’re writing shit like it is.
Peace up.

Damn it!
They kicked the dinner table over, using the temporary cover it provided to dive behind the couch.
Not that a couch or a table would really stop a bullet. It was more for the visual cover than anything. Harder to hit a target you can’t see. It would give them a chance to reload, at least.
They didn’t feel too bad about destroying the homeowner’s furniture. The room was a mess anyways. The picture hanging on the wall wasn’t even straight.
“Vanderwood?” The voice in his earpiece had a forced calm, like it was trying to sound professional. The hesitant rise in pitch at the end of their name gave them away.
Well, to them, at least. Vanderwood was pretty familiar with that voice by now. Just like they were starting to get familiar with the exact angle that stupid picture was off balance. Seriously, it was so uneven. How did the guy who lived here not notice?
“What is it, Zero Seven?” they asked, peeking out quickly from the couch to fire back at their attackers. They heard one fall to the ground with a pained shout.
“Did you find the papers?”
Vanderwood sighed. Well, half sigh, half groan of annoyance. If they’d found it, don’t you think they’d have left before people started shooting at them?
“Our info must have been bad, there’s nothing like that here.”
“Oh.” An awkward pause, broken up only by a faint clicking of keys. “You should probably get out then. Backup’s on the way. For them, not you. That’s what I was calling to tell you.” The kid was clearly nervous, which only confirmed the depth of shit Vanderwood was currently in. Great. “You should probably get out of there fast.”
“Thanks, genius.” Vanderwood usually tried not to be too harsh on the kid, but they were a little stressed about the whole imminent death thing.
…And it didn’t help that the stupid slanted picture was staring at them. Crookedly.
“Any chance of backup?” they asked, staring back at the offending picture.
“No one else is in the area…” They could hear a faint tapping in the background. 707′s nervous habit. “They’re coming in from the north, so you should head for the back door.”
“Understood.”
The line stayed open for a second longer, as though the kid wanted to say something else. But he didn’t. Vanderwood heard a ‘click’ as the line went dead, and turned back to face the problem at hand.
That stupid picture.
What? No! The whole ‘being trapped’ thing. That was their priority. Stop looking at the picture. Stop it.
They leaned out and shot at the doorway where the three men (two? They didn’t know if the one they’d shot was still there) were hiding, forcing them back into cover. They took the opening to run for the kitchen.
Right past the crooked picture.
It was so close. It was literally right there. If they just… reached out… and pushed it just a little…
The picture was straight. All was well. They had a bullet in their arm.
…wait, what?!
They dove into the kitchen and kicked the back door open. They didn’t have time to stop and worry about the fact that their arm was slowly bleeding out.
Should’ve thought of that ten seconds ago, huh idiot?
They ran outside, launching themself over the fence. Before they left, they turned back and lodged their taser against the fence, wedging it in so the wires held the trigger down. Let’s see those assholes climb the fence now.
They continued to follow 707′s directions until they finally met up with their getaway driver. Which was… 707 himself. They hadn’t expected that.
“Are you even old enough to drive?” they joked weakly, all but collapsing into the passenger seat.
The kid’s eyes widened, honing in on the blood seeping through the older agent’s sleeves. “What happened?”
“I… got distracted.” They felt their face heat up as they remembered their temporary lapse of judgement. Was it too much to hope 707 wouldn’t ask?
“By what?”
Of course it was.
“There was this picture on the wall and it was just… so crooked.”
Saying it out loud only made it sound worse. They regretted it instantly. They didn’t owe this kid an explanation. Why didn’t they just lie?? They blamed it on being tired.
The heavy silence after their explanation only lasted a moment before it was ripped apart by 707′s barely contained laughter.
“You didn’t.”
“Shut up.”
“In the middle of a firefight? Seriously?”
“I said shut it!”
And although the laughter eventually faded, they both knew that Vanderwood was never going to live this down. Ever.
Can I get a matchup?^^;Im a rather calm person,with a bit sassy sense of humor.I'm understanding toward others,but more strict toward myself.Daydreaming&saving energy a lot.I hate mess&tend to remind people to take care of themselves a lot if they need it,but I'm not the best example of it even if I try hard;;I love gaining new knowledge,I'm into art as well.Ambitious but lazy tho;;I get scared easily.I dislike doing risky things,but if I feel stressed,I tend to act carelessly.Have a nice day^^;
I match you up with:
Jumin Han
He likes that you’re not a risk taker. Honestly, it relaxes him so much because he knows then that you are less likely to get hurt, and he couldn’t bare for you to get hurt.
He loves that you’re calm but that you can also be quite sassy. He just finds it very refreshing and, to him, it feels like you’re trusting him and opening up. He also likes that you’re understanding, because Jumin will need help and patience to get better. Your little reminders for him to take care of himself makes his day.
However, he will beg you to take care of yourself and to be understanding and patient with yourself, and he will definitely care for you and he’ll be incredibly patient, gentle, and kind with you.
He has maids on hand so there will never be a mess, and he’s quite meticulous himself, doing everything to prevent a mess ever being made in the first place. He also has cooks/chefs so there will never be mess in the kitchen either as they take care of it all themselves.
He likes that you’re ambitious, and he will protect you from anything scary, not particularly being big on scares either unless it’s you messing about and trying to scare him.
I hope this is okay, and take care!
This was a request and at first I wasn’t sure if I had anything to provide with, but as it turn out it got a little longer than I expected because there were actually things I had to say!! Wow!!
Anyway, this is some guidelines I follow when I try to make the face expressfull, more specifically the mouth! It is often neglected, since it’s actually pretty hard, I’ll admit. But I’m here to help (hopefully…)! A mouth expression tutorial as per request. Enjoy and hopefully it will help some a little. ʕ•ᴥ•ʔ
This is super important. The upper jaw follows the angle of the head, and the lower jaw will depend on how open it is. Make sure you have a rough estimate of where the teeth are, and how much of them you’re going to see!
The lips will VERY roughly follow the same angle as the teeth. It really depends on the character, but it gives you a sense at least.
If you DON’T do this, you’re going to lose so much volume and the mouth is going to end up looking unrelatable. I showed this example in this tutorial:
The cheeks, chin, and tongue play a role too!
Try look at your own mouth or references! I have a very pliable and large mouth, so that’s one reason why my characters have it too lmao.
I cannot emphasize how important asymmetry is when drawing expressions. It applies not only to the eyebrows to achieve the Dreamwork Face™, but also the mouth. Seriously if you draw a symmetric mouth I will deliver myself to your mailbox and then shout at you until you fix it.
Look at the difference between these two for example: which one has more “life”?
I think you get the idea.
Here’s an old drawing I have but it illustrates how I think when I squish the mouth, and use folding and wrinkles to my advantage.
Look at your own face and see where skin bundles up, where it creases the most and when bumps appear on your chin. Subtle details makes all the difference!
One VERY effective detail is illustrated in the first sketch, where I pull upwards on one side, and downwards on the other. That’s a good detail to use when the character is making a skewed expression, or is extremely frustrated. I encourage you to play around with that concept bc it’s ~super effective~!
Happy: Your entire mouth is pushed upwards, not just the corners of your mouth!
I tend to draw a :3 mouth bc I’ve been drawing Lance too much….. You don’t have to but it’s basically imprinted in my motor memory by now.
Pouting/frowning: corners are pushed down, middle pushed slightly up. Sometimes, there’s a slight dip in the middle too. It can give a sense that the character is biting their lips.
Showing frustration/intimidating/is intimidated: basically showing a lot of teeth. The corners are as open as possible and the middle sorta more squished. An extremely important detail here is showing some of the gums, and open space between the cheeks and teeth. That way it looks like the mouth it open to it’s full potential. Here is also where you basically MUST add folds and bumps, or else it’s not going to look relatable.
(Here I am again with the pulling upwards on one side and downwards on the other, as illustrated on the last sketch)
And then again, here’s just another doodle showing how important it is to show the gums. It’s the same face twice, but the second one looks slightly more frustrated doesn’t it?
(from my other tutorial on how to draw facial expressions)
As you can see, this last one is very versatile and I draw it a lot. Play around with the basic shape and see how much subtle details makes a lot of difference!
I hope that cleared some things up and was somewhat helpful! Enjoy drawing ✨
